HRH The KING 0 Posted February 2, 2005 (edited) If NASA really wanted to be accurate they should have written this on the plaque: "A memorial plaque to the victims of the Columbia disaster who were killed when a large piece of insulation foam from their external fuel tank hit the leading edge of the left wing cracking it and allowing hot gases to melt the inner structure of the wing causing the shuttle to break up upon re-entry and sending the seven astronauts to a fiery terrifying death. We saw the insulation hit the shuttle, we had a discussion about it, we reviewed the tapes, we launched an investigation, heck, it even happened before to other shuttles on lift-off, but we didn't even warn the astronauts and have it checked out. HRH The KING 0 Posted February 5, 2005 Well, it's true that government interference (the Dept. of Defence in particular) definately contributed to the very weak design of the shuttle launch system and orbiter, but the two disasters were clearly caused by NASA ineptness when it comes to making decisions.
